The forums
Post a new thread in the Report A Cheater Forum with the relevant server and date in the title. Then, simply copy and paste this form into the message body:

SteamID:
Hack:
Comments:
Regs:
Please be as detailed as possible. Any extra information can be included in the comments section e.g. screenshots, demos, other reasons for banning such as racism e.t.c. Also remember to include the names of server regulars who can verify your report.
